Order: 

 

Registered Nurses (RN) and Registered Practical Nurses (RPN’s) will initiate:

  • 10 mg Warfarin Nomogram (the nomogram attached, as appendix A, is used to start patients on warfarin)

  • adjust warfarin dose to maintain a therapeutic target International Normalized Ratio (INR) after the patient has been anticoagulated for a minimum of 2 weeks by the physician

  • adjust warfarin dose as per Appendix B, for both new and long term patients.
